You may Report a Pothole by calling our Pothole Hotline at 815-987-5771 (Hearing Impaired 987-3083) or by using the form below.  Please provide accurate contact information so we may contact you if further information is needed.  Please note that if you do not fill in your address, phone number, or email address, and for some reason, the City can not complete your request, we have no way of following up or notifying you of the status of your request. Contact information is not required, but it is recommended that some information is left so, if necessary, City staff can follow up with you.
